Is your child interested in participating in sports next year? If so, (s)he needs a physical, completed on the DIAA Physical Form, done prior to participating in any athletic activities…..This includes Try-Outs!! The form is linked below, but your doctor’s office may already have copies. This form is a STATE requirement for all scholastic athletes in Delaware. Any physical exam done on April 1, 2016 or later is good for the 2016-17 school year.
Physicals will be offered at school on Tuesday, August 31. The cost will be $25, CASH ONLY.
In order to be considered for a position on any sports team, each student must have the Colonial School District Sports Permission Form and the DIAA Physical Form (dated after 4/1/16) on file with the Athletic Director, Mr. Sawdey. All pages must be signed & dated where indicated. The DIAA forms may be found by using the link on this page.
Participation Forms must be completed by parents/guardians prior to the Physical exam. Packets are available from Mr. Sawdey or from the Main Office or Central Office. You can also find the forms here.
