Health Care Information:
Attach is a very important email regarding your benefits as of July 1, 2017. To avoid unpaid and delayed medical bills processing, please take a few minutes read the attachment in its entirety (see below). The email explains a few requirements in accordance to your health plan – Aetna HMO. For example:
- All Aetna HMO members should adhere to the following:
- You must have a designated Aetna Primary Care Physician (PCP). To select or change a PCP, please call Aetna at 877-542-3862. Please make sure your physician is assigned PRIOR to receiving services.
- It is your responsibility to ensure that your referral for all specialists are updated PRIOR to your visit.
- All Aetna members must use Quest Diagnostics for all lab services. To find a Quest Diagnostics near you, call Aetna at 877-542-3862.
Again, read the attached letter in its entirety to ensure all bills are paid timely. In addition, please store this letter so you can reference it at a later date.
Local Benefits Information:
As you know, July 1st starts our new benefits year with the new local benefits. Please remember as of July 1st our local benefits are as follows:

You do not need a card to receive services for local benefits. To reference your account, provide your provider with your ID Number. Your ID Number for vision and dental is your employee id number proceeded with three zeroes. For example, employee ID 123456, the ID number will be 000-123456. Your employee ID number is located on your paystub. Please remember Colonial School District does not provide social security numbers to the Vision and Dental providers. Please do not share your personal information.
