Special Dietary Information

Colonial School District accommodates all special dietary modifications for all students. Parents of students with food allergies must contact the school nurse and fill out the Prevention and Emergency Response Plan for Students with Allergies. This form must be filled out and signed by a state licensed health care professional, and returned to the school nurse. 

Helpful Documents/Forms for Parents:

Pending appropriate signed documentation, the below accommodations are made.

  • Dairy Allergy – All cheese, yogurt, and other items containing milk, whey, casein will be eliminated and substitute with an alternate source of protein.
  • Lactose intolerant – Lactaid and Soy Milk available for students who fill out the Milk Alternative Form and return to the school cafeteria, school nurse or Nutrition Services office.
  • Gluten/Wheat – All wheat/rye/barley will be eliminated and substituted appropriately.
  • Soy – All items containing a soy protein, soybean oil or soy lecithin will be eliminated and substituted appropriately.
  • Egg – All items containing egg/egg white/egg yolk will be eliminated & substituted appropriately.
  • Tree Nuts & Peanuts – Nutrition Services does not serve breakfast or lunch items that contain tree nuts/peanuts in Elementary Schools. Some food items are processed in a facility that also processes tree nuts/peanuts. Refer to the Classroom Celebration Item List. 
  • Fish & Shellfish – Nutrition Services does not serve any shellfish. Fish sticks are offered approximately once per month. Fish sticks are made from 100% Pollock. Tuna Fish sandwiches are served approximately once a week.
  • Sesame – All items containing sesame will be eliminated & substituted appropriately.

To file a program discrimination complaint, complete the USDA Program Discrimination Complaint Form, AD-3027, found online at How to File a Program Discrimination Complaint and at any USDA office or write a letter addressed to USDA and provide in the letter all of the information requested in the form.
